California Consulting (Synergy Wellness Collection Point) – Patient Agreement
This Agreement (“Agreement”) is entered into between California Consulting (Synergy Wellness Collection Point) (“Synergy Wellness”) and the individual (“Patient”) seeking to collect prescribed or recommended cannabis products for medical and/or wellness use.
By completing the online registration form and ticking the acceptance box, the Patient confirms understanding and acceptance of the terms below.
1. Purpose
Synergy Wellness operates as a wellness-focused collection point facilitating access to medical-grade cannabis products for patients who have been recommended or prescribed such use by a healthcare professional. This Agreement outlines the terms under which Synergy Wellness provides collection, advisory, and related wellness services.
2. Eligibility
To register, Patients must:

Be 18 years of age or older.

Provide valid identification.

Provide medical documentation when required, supporting their use of cannabis for therapeutic or wellness purposes.
3. Nature of Service
California Consulting does not sell cannabis directly.
It provides:

Collection and logistics services,
-
Coordination with verified suppliers,
-
Compliance oversight within South African law.
All products collected are for lawful personal medical or wellness use only.
4. Confidentiality & Data Protection
Synergy Wellness complies with the Protection of Personal Information Act (POPIA). Patient data:

Will be collected, stored, and processed securely,
-
Will not be shared without consent, unless required by law.
5. Patient Responsibilities
By registering, the Patient agrees to:

Provide truthful, accurate information.

Use products responsibly and only for personal, lawful medical/wellness purposes.

Not resell, transfer, or distribute products obtained through Synergy Wellness.
6. Limitation of Liability
California Consulting / Synergy Wellness will not be liable for:

Misuse of collected products,

Adverse reactions,
-
Improper administration or storage by the Patient.
Patients are solely responsible for following medical guidance from licensed healthcare professionals.
7. Termination
Either party may terminate this Agreement at any time by written notice.
Upon termination, any in-progress collections must be finalized according to Synergy Wellness policies.
8. Governing Law
This Agreement is governed by the laws of the Republic of South Africa, and all disputes fall under South African legal jurisdiction.
9. Data & Digital Consent Clause
By ticking the acceptance box on the digital registration form, the Patient:

Consents to the collection, storage, and lawful processing of personal and medical information in compliance with POPIA.

Understands that such information is used solely for:

Patient verification,
-
Wellness coordination,
-
Communication regarding orders, appointments, and service-related notifications.

Consents to being contacted by:

Email,
-
SMS,
-
Phone,
-
WhatsApp (where applicable),
for service-related messages.

Acknowledges that Synergy Wellness is not a medical practice and:

Does not diagnose conditions,
-
Does not replace qualified medical consultation,

Advises Patients to seek medical guidance from licensed healthcare professionals.

Understands that clicking “I Accept” legally constitutes:

A valid digital signature,
-
Confirmation of full reading and acceptance of this Agreement.

Digital Acceptance
By ticking the checkbox and completing the registration, the Patient confirms:
